Benefits of Case Management

I have been a case manager for six years now, and if you were like me many years ago, you are probably thinking, what the heck is case management?

Case management is an incredibly beneficial service that connects clients to resources, valuable services, and other providers. You collaborate with passionate and resourceful humans who dive into your individual situation to determine how they can best support you. The most common among our clients is support in housing, employment, food assistance, linking to transportation, budgeting, connecting with local resources, applying for assistance programs, and social support. Transportation is not a barrier to receiving case management services. We meet clients in their homes, at our offices, or within the community, whichever suits them best.

Unexpected times happen, and many have found themselves on the verge of being unhoused. We can help people access resources to avoid that and obtain safe and stable housing.

Finding employment and creating a resume can feel like a stressful process, but it doesn’t have to be. We can be of additional support during the process. We can assist with searching for employment based on your needs and preferences, creating a professional resume, practicing interview skills, and more.

With the increase in inflation, many feel its effects at the grocery store. We can connect individuals to local food resources and assist them in applying for food assistance programs.

Many within our community struggle with access to transportation. We can assist with anything from studying for your permit test to linking you up with local transportation within your community. Many with Medicaid do not know they have coverage for their doctor visits through their insurance.

If you find yourself stressed about money and want to learn more ways to manage your money, we can assist with your specific needs, such as creating a budget and learning ways to manage your money based on your and your family’s needs.

Many families fall on hard times, but often, they don’t know the organizations that can assist them. We can be an excellent support in locating organizations that can meet your needs during difficult times and assist in the application process.

You may find yourself isolating and feeling anxious to get back out into the world or not knowing where to start. People have been feeling this even more since the pandemic. We can ease those nerves by providing support in public spaces or within your home to build your confidence in socializing, and we can connect you with local groups that share the same interests as you or be collaborative care if you see a therapist as well.

One of the best parts about case management is that we are there to meet you where you are and help you create goals to get you where you want to be with extra support. If you need extra support, do not hesitate to ask about our case management services! We have an excellent and passionate team waiting to assist!

“Having a case manager has been great. I don’t have a support system and struggled with getting out of the house without having a panic attack. I had to quit my job due to my declining eye health and was struggling with having enough money to cover my bills. Through case management, I have obtained social security disability, food assistance, and support with starting therapy, and I no longer have panic attacks or anxiety leaving the house. Now, I look forward to being out and about. The extra set of ears to vent to helps me work through decisions or struggles. I am extremely thankful for my case manager.”

“I have trust issues, and it takes me a long time to open up. I found myself at ease speaking with my case manager very early on. I felt comfortable being myself around them. I am working on getting my driver’s license and my first job. I failed my permit exam three times before meeting my case manager. My case manager and I created note cards that I study at home, and I recently got the highest score on my exam. My case manager helped me make a resume and look for jobs in my town. I am not good with using the internet, but they have taught me how to navigate it so I can look for resources on my own. I like that they can talk to my therapist and that my case manager knew of my anxiety about meeting new people before we met. I struggled with anxiety around new people and in public places. I like that my case manager gives me homework to do, which helps me feel more comfortable when I am out at the grocery store. I finally feel like I am more independent and able to do things on my own. I recently started walking to the local store and shopping on my own.”
