At Insight, our Case Management Team focuses on helping others help themselves. Sometimes that means being an advocate, at times that means providing access to important resources and other times it means being a voice of empowerment. Case Management aims to provide specific, measurable and individualized services to each person served to help you live your best life.

Working collaboratively with therapists and other care providers, we develop a plan that focuses on the individual’s ability to succeed in the community, identify and access needed services, show improvement in school, work, family and integration and contributions within the community.

With the goal of achieving personal independence we provide opportunities for individuals to practice skills learned in therapy and grow.
